VETERANS ADMINISTRATION (VETERANS BENEFITS)

The Missouri Approving Agency for Veterans’ Education approves Midwest University for the education of veterans and their eligible dependents, National Guardsmen, and selected Reservists. This approving agency has APPROVED a course(s) offered by Midwest University for the education of veterans or eligible persons under the provisions of Section 3676, Title 38, United States Code, and Title 5, Missouri Code of State Regulations 30-4,020.

The Veterans Administration determines the eligibility for specific benefits. Students may obtain application forms by contacting the Registration Office of Midwest University or Department of Veterans Affairs Education Customer Service Office at 1-888-442-4551. The operator will connect you with your respective state’s Department of Veterans Administration Regional Office for assistance.

Applying for Benefits

1.	Submit an application for benefits to the VA Regional Processing Office: VA Regional Office, P.O. Box 66830, Saint Louis, MO 63166-6830.
	You can do this:
	a.	Electronically through the Department of Veteran Affairs VONAPP website.
	b.	With this printable application for GI Bill Benefits (VA Form 22-1990)
	c.	With this printable application for Survivors' and Dependents' Educational Assistance (VA Form 22-5490)
	d.	With this printable application for a Change of Program or Place of Training (VA Form 22-1995)
2.	Submit a completed 'VA Benefits Certification Request' to the Midwest University Office of Financial Aid.
